After you have performed the required reading, your parent/guardian will need to complete the form below so we know where to send your reading certificate and reward. How to Play Book titles may be used only once per card, and only one entry per person. For middle grade and young adult readers. How do I win? BINGO: Complete a horizontal, vertical or diagonal line! Turn in your card and you’ll be entered into a drawing for a gift card! BLACKOUT: Complete all 25 squares! Turn in your card and you’ll be entered into a drawing for a gift card to an independent bookstore! How do I submit my card for the prize drawing? Submit the front and back of your completed card (with either a bingo or blackout) in only one of the following ways: DROP OFF your card (or a copy) at any branch of The Seattle Public Library. COMPLETE the submission form . GIVE your card (or a copy) to an SPL librarian at an outreach event. Entries must be received by end of day on Tuesday, Sept. 3, 2024. Do not a submit card more than one time—Thank You! What are the prizes? Readers who complete a horizontal, vertical or diagonal line will be entered into a raffle to win a $25 Visa gift card. There will be one winner per branch. Readers who complete a blackout will be entered into a raffle to win a $100 gift card to Elliot Bay Book Company.
